Support Coordination in Western Melbourne

Help making sense of an NDIS plan, connecting with the right services, and putting supports in place with confidence.

Overview

What it is

Support Coordination

Support coordination is funded help to understand your NDIS plan, find the right services, and put your supports in place. It is the bridge between a plan on paper and a life that actually works. For many participants and families, the plan itself is only half the battle. The other half is figuring out how to use it.

An NDIS plan can be confusing, even for people who have had one for years. The budgets have names that are not always clear. The funding can be split across Core, Capacity Building, and Capital, each with its own rules. The providers all have different processes, waitlists, and ways of working. And on top of all that, the participant is often trying to manage their own health, their family, and their daily life at the same time.

A support coordinator takes that weight off. We help you read the plan, understand what each budget is for, and work out how to use it in a way that actually fits your goals. We connect you with providers, help you compare options, and make sure the different pieces of your support work together rather than pulling in different directions.

There are different levels of support coordination. Standard support coordination focuses on connecting you with services and building your capacity to manage your own supports over time. Specialist support coordination is for participants with more complex situations, often involving multiple services, health needs, or challenging circumstances. We help you understand which level you have funded and what it can do for you.

Support coordination sits under the Capacity Building budget, and it is time limited. The aim is not to keep you dependent on a coordinator forever. It is to build your confidence and skills so that, over time, you can manage more of your own supports, or at least know exactly where to turn when you need help.

We see support coordination as a partnership. We bring the knowledge of the system and the local provider landscape. You bring the knowledge of your own life, your goals, and what matters to you. Together, we turn the plan into something that genuinely works for you and your family.

Who it suits

For a participant or family who would like guidance understanding their NDIS plan, finding providers, and setting up supports without feeling overwhelmed.

Our Approach

How Mirai delivers it

We start by getting to know you, not just your plan. What are you hoping to achieve? What is working well in your life right now, and what is not? What has been frustrating about the NDIS so far? What does a good week look like for you? These conversations shape everything we do, because support coordination only works when it is built around your actual life.

Then we look at the plan together. We explain what each budget is for, how much you have, and what it can and cannot be used for. We do this in plain language, without jargon, and we are happy to go over it as many times as you need. There is no such thing as a silly question, and we would rather you ask than stay confused.

From there, we help you find and connect with providers. We know the western Melbourne landscape, so we can suggest options that are local, reliable, and a good fit for your needs. We help you compare them, understand what they offer, and make choices that feel right. We do not push you toward any particular provider. The choice is always yours.

If you have multiple supports in place, we make sure they are working together. That might mean coordinating between your support workers, your allied health, your school or workplace, and your family. When the pieces are coordinated well, the whole system runs more smoothly and you get more value from your plan.

We also help you prepare for plan reviews. When your plan is coming up for renewal, we work with you to review what has worked, what has not, and what you might need next time. We help you gather the evidence and write the goals in a way that gives you the best chance of getting the funding that fits your life.

Throughout all of this, we are building your capacity. We want you to feel more confident managing your own supports over time. That might mean showing you how to use the NDIS portal, helping you understand your service agreements, or just being there to answer questions when you are unsure. The aim is independence, not dependence.
